Things to Remember Prior to Your Appointment at Carolina Spine & Hand, PA

If required by your insurance company, referral forms and authorizations must be received form your primary care physician prior to your appointment.

Complete new patient forms prior to arriving for your appointment (these may be downloaded from our website), or arrive 30 minutes early to complete the information.

Established patients should notify our receptionist of any changes in insurance, address or contact information. Please bring your insurance card with you to each appointment.

Always bring a list of current prescription and over-the-counter medications you are currently taking.

Please bring with you a photo ID, insurance cards and co-payment. 

Please provide 24-hour notice if you need to reschedule an appointment.

Emergencies and Prescription Refills

If you need assistance during regular office hours, please call our office and you will be given instructions based on the nature of your call. If you are a patient of record and need assistance after hours, please contact Rowan Regional Medical Center at 704-210-5000 and ask for the physician on call.
